As a member of a health plan managed by American Health Group, great service and valuable information is easily available to you. Health care is important to us, and the services provided below are designed to ensure that your health care experience with American Health Group is a positive one.

Contact Customer Service
You will find phone and e-mail contact information for American Health Group's Customer Service department, hours of operation, and forms available for download.
Search for a Provider
It's important to maximize your health plan's benefits by selecting physicians and hospitals that are a part of your company's health care network. If you are an existing member, you may search for a physician or hospital that is located close to your home or work, and that is in your specific network.
Request a Health ID card
You can assist your health care provider by always carrying a current health ID card that identifies you as a covered member. By doing so, you ensure that your company's health benefits are applied for maximum benefit. If you are an existing member, you may request a new or additional ID card.
