	Please Note: American Health Group (AHG) contracts with many local, regional, and national networks. Therefore, if your practice is affiliated with one of these networks, you will need to contact that network in order to submit updated information.

	Written Notification

The following data changes require written notification to our Managed Care Department:

Tax Identification Number (TIN). Changes must include copy of W-9 form
Retirement
Disenrollment
Deceased Provider
Malpractice Changes on the malpractice face sheet
National Provider Identifier (NPI)
Other changes to demographic information (address, phone number, etc.)
