Please Note: American Health Group (AHG) contracts with many local, regional, and national networks. Therefore, if your practice is affiliated with one of these networks, you will need to contact that network in order to submit updated information.

How To Submit

If you are part of American Health Group's Managed Care Network and wish to update the demographic information in your provider profile, you may submit these changes in writing by:

Written Notification

The following data changes require written notification to our Managed Care Department:
  • Tax Identification Number (TIN). Changes must include copy of W-9 form
  • Retirement
  • Disenrollment
  • Deceased Provider
  • Malpractice Changes on the malpractice face sheet
  • National Provider Identifier (NPI)
  • Other changes to demographic information (address, phone number, etc.)
  Maintaining accurate data is important to AHG. Your assistance in this process is appreciated.
