The following data changes require written notification to our Managed Care Department:
- Tax Identification Number (TIN). Changes must include copy of W-9 form
- Retirement
- Disenrollment
- Deceased Provider
- Malpractice Changes on the malpractice face sheet
- National Provider Identifier (NPI)
- Other changes to demographic information (address, phone number, etc.)
