The Health Insurance Portability and Accountability Act (HIPAA) of 1996
is legislation passed in response to requests for standardization of the
healthcare payment process and privacy aspects of releasing and
protecting personal health information (PHI).
American Health Group is in full compliance with HIPAA requirements. At
this time, AHG is committed to its "Spirit of Excellence" standards for
compliance with the Privacy and Security Standards and the Transaction and
Code Set Standards.
HIPAA will continue to be a permanent component of AHG's healthcare
strategy.
American Health Group's "Privacy Policy" applies to personal health
information which is maintained and/or transmitted in any form, including
electronic, paper, or oral discussions, that is identifiable to an
individual. Personal Health Information (PHI) is Confidential
Information. Since AHG is a medical management organization with a
claims administration division, PHI is a vital component of our healthcare operations. AHG has administrative, physical, and technical
safeguards in place to protect the data integrity and confidentiality of
your personal health information.
The confidentiality of your personal health information is our primary
focus. AHG's Privacy and Security Officials have taken many steps to
prevent the unauthorized release or disclosure of personal health
information. Our employees have undergone mandatory HIPAA Awareness
Training sessions and are also held accountable for internal HIPAA
compliance.
In performing their duties, our employees need to access confidential
information regarding our clients. Access to confidential information is
limited to those employees who need that information in order to perform
their job requirements. In other words, access to PHI is on a "need to
know" basis. All uses, disclosures of, or requests for PHI will be limited
to the minimum amount necessary to accomplish the intended purpose, based
on the employee's professional judgment. By completing the HIPAA Awareness
Training sessions and signing the Employee Confidentiality Agreement, the
employee certifies their understanding that PHI is considered
confidential, that it is important to safeguard PHI, and that AHG will
take appropriate action against any breaches to the HIPAA standards.
American Health Group may use certain methods to collect other kinds of
information that cannot be personally identified with you, including
"cookies," "referrers," IP addresses, and environment variables.
A "cookie" is an element of data that American Health Group can send to
your browser, which may then store it on your system. It can be used to
provide you with a customized user experience. The American Health Group
website may use "cookies." We treat any personally identifiable
information with the same confidentiality as when you went through your
company enrollment. You can set your browser to notify you when you
receive a "cookie," giving you the chance to decide whether or not to

System information that American Health Group gathers includes time,
