member select a primary care physician (PCP) or seek care from a
physician, specialist, facility, or other healthcare professional in the
network, either directly or by referral from another healthcare
professional, you are seeking care from that healthcare professional and
not from American Health Group. Your physician and other healthcare
providers exercise independent medical judgment and should discuss
treatment options with you, even those options that might not be eligible
for benefits under your Certificate of Coverage or Group Plan. American
Health Group is not responsible for, does not provide, and does not hold
itself as the provider of your medical care.
